Roster

The roster file will contain all current licensees. This file is a CSV (Comma-Separated Values) text file that contains a header record denoting the field names. The following fields are included for each record. Please note that the address listed is for the licensee's primary practice address. If no primary practice address is listed, the licensee did not provide one to the board.

There is a $300.00 fee to receive a roster. After successful payment, you will receive an email containing the roster file.

NOTE: Data provided is for practitioners currently licensed as of the date the roster is requested.

Purchaser understands and agrees that no part of this data will be distributed in any form or by any means for the purpose of resale. Purchaser understands and agrees MSBML data represents unpublished data which is to be provided to Purchaser solely for information. Any other use of data obtained by Purchaser shall constitute unlawful usage by Purchaser. Purchaser shall not incorporate MSBML data with or into MSBML data in the same data field nor incorporate MSBML data, as a whole or any portion thereof, with or into MSBML data to an extent that it is impracticable to extricate the profiles or any portions thereof from the MSBML data at any time. MSBML data is provided "AS IS" without warranty of any kind as to completeness or accuracy. However, MSBML will make reasonable efforts to keep the data current and complete. In no event will MSBML be liable for any damages, including any lost profits or other incidental or consequential damages, arising out of Purchaser's electronic access to, use, nonuse, misuse or interpretation of or inability to use MSBML data even if MSBML has been advised of the possibility of such damages.
